Propelled Brands Data Subject Access Request Form

Residents of certain states may be entitled to request access to and/or deletion of personal information that we hold about you. Complete the form below to submit your request. When we receive your information, we’ll use it to verify your identity and review your request. You can only submit one type of request at a time. Need to make more than one request? • Ask that we delete the personal information collected about you.

Making a Request

A valid email address is required to complete your request online. For each request, we’ll send a verification email to the email address you provided. If you don’t receive a response within 72 hours, check your spam folder. You’ll have 3 days to verify your email before it expires. If not, you’ll have to submit another request.

If you are requesting on behalf of another person, please send the request to privacy@propelledbrands.com and include the following information about you and the person on whose behalf you are making the request: full name, mailing address, email address, and phone number. You should also provide proof of your authorization to act on the other person’s behalf. If you are a business acting as an authorized agent on behalf of a consumer, you must provide proof that you are registered with the California Secretary of State. We will contact you for additional information once the request has been received.

After we process your request to remove your personal information, you may see advertisements regarding our products and services. We may advertise to a general audience or place advertisements on websites and other digital entities. For example, if you visit an office interior website, you may see advertisements that promote our products and services.
